The decrease in bone mineral density may be due to decreased bone resorption due to decreased serum levels of osteocalcin.[61] Several published trials have demonstrated that a single high dose of corticosteroids in children or adolescents may lead to significant bone loss.[62,63,64,65,66,67,68,69,70,71,72,73,74] In one randomized controlled trial, patients receiving dexamethasone plus prednisone had a mean bone loss of 2.1- to 3.3-cm2 with a mean increase of 0.7- to 0.8-cm2 with steroids alone. This may be due to the fact that patients in this study are adolescents. [68] In contrast, the effects of dexamethasone in these studies were nonsignificantly smaller than those noted with prednisone alone. Pediatric use of oral prednisone can lead to changes in serum oestrogen, including high levels. This includes a significant rise in estrogen receptors (ERs). These changes in oestrogen may contribute to osteoporosis, although the exact mechanism remains uncertain.[69] Oral prednisone has a negative effect on bone as measured by a fracture rate of osteoporotic fractures (HR-OR=0.98 for a 4.5 year interval, 0.86 for the next 4 years). The rate of hip fracture was unaffected by the use of this medication in healthy patients, but at least 1.2 fractures occurred in patients receiving oral prednisone compared with no fractures in those who were treated with placebo.[72] Cortic Related Article:
